Article 1 – Legal definition

Article 2 – The processing of personal data

Personal data means any information relating to an identified or identifiable natural person (‘data subject’).

An identifiable natural person is one who can be identified, directly or indirectly, in particular by reference to an identifier such as a name, an identification number, location data, an online identifier or to one or more factors specific to the physical, physiological, genetic, mental, economic, cultural or social identity of that natural person.

We process the following categories of personal data from you after you have registered to your secure HelloDoc Healthspace account via www.hellodoc.uwzorgonline.nl

Article 3 – purpose of processing

We do not collect your personal information without a purpose. Your personal data are processed:

to provide you with our telehealth advice service

to comply with the medical legal obligation between a doctor and a patient

Article 4 – Registration of personal data

Your personal data are recorded in an (electronic) register hosted by pharmeon.nl and promedico-asp.nl.

Article 5 – Your rights regarding information

Pursuant to Article 13 paragraph 2 sub b GDPR each data subject has the right to information on and access to, rectification, erasure and restriction of processing of his personal data, as well as the right to object to the processing and the right to data portability.

You can exercise these rights by contacting us at support@hellodoc.health.

Each request must be accompanied by a copy of a valid ID, on which you put your signature and state the address where we can contact you.

Within one month of the submitted request, you will receive an answer from us.

Depending on the complexity and the number of the requests this period may be extended to two months.

Article 6 – Legal obligations

In case of infringement of any law or regulation, of which a visitor is suspected and for which the authorities require the personal data collected by the collector, they will be provided to them after an explicit and reasoned request of those authorities, after which these personal data do not fall anymore under the protection of the provisions of this Privacy policy.

Article 7 – Commercial offers

You may receive updates on our services. You will be required to grant permission to send you this information to your provided email address. If you do not wish to receive them (anymore), please send us an email to the following address: support@hellodochealth.nl.

Your personal data will not be used by our partners for commercial purposes.

The collected data are used and retained for the duration determined by law. This is a minimum of 5 years for medical related information.

Article 9 – Applicable Law

These conditions are governed by Dutch law. The court in the district where the collector has its place of business has the sole jurisdiction if any dispute regarding these conditions may arise, save when a legal exception applies.

Through our services, HelloDoc Health collects certain personal information

(a): Information that you as our clients have provided us

You may provide details about yourself that are personal and related to the medical issues you have when you register with our services. In the course of any contact via email, chat message or video consultation, this is solely done on a platform www.hellodoc.uwzorgonline.nl that ensures secure and confidential handling of your details for the purpose of the services. Upon registration on our Hellodoc Healthspace, you will be asked to provide certain personal information such as your name, email address, occupation. On our website you will be asked to fill out a form and provide your name, telephone number, email address and the type of mobile services you wish to request.

You will also be asked to provide payment and billing information such as your credit card details, bank details and billing address. This information will be requested by a third-party (Paypal). This third-party also has their own privacy policy which you can read here.

During the time that you are our clients, for all medical issues, HelloDoc will keep all files of a summary of your medical complaints, treatment and follow-up records.

We protect all your personal and medical information.

(b): information we collect automatically through our services

When you use our services, we may collect certain information automatically about you in order to provide the best care. This may include but is not limited to previous medical history, medication use, family history, allergies and so forth. We collect this information for the sole purpose of treating the condition with which you presented.

Contact forms and communication

When you send us a form, email or communicate with us through our website, it is very possible we will keep these messages. During our contact with each other we may ask personal questions which are relevant to providing the care and optimal service we strive for. These questions will be necessary to help you towards a healthy recovery path. Furthermore we will save this information in our secure web servers or in that of our third-party server (www.hellodoc.uwzorgonline.nl). We will not share or combine your information with third-party sites pertaining to your medical conditions, treatment, consultations in general.
